Funding Resources

We understand that many with disabilities face financial challenges. Below we have listed community, state and national funding sources that you may qualify for to assist you.

Children's Miracle Network

Children’s Miracle Network has been assisting children in the greater La Crosse Area for over 30 years.  They serve children until the are 18 years of age and until 21 years of age if they are attend extended education.

IRIS (include, Respect,I Self Direct)

IRIS (Include, Respect , I Self Direct) is a state run Medicare program for elderly and disabled adults. In consultation with your IRIS team you may be able include STAR in your Care plan.

To enroll in IRIS, you must:

  • Live in Wisconsin
  • Be at least 18 years of age
  • Meet certain Financial and financial criteria
  • Be an Adult with disability
  • Not live in a nursing home

You should contact the Aging and Disabled Resource Center (ARDC) to discuss if you are eligible and your options. If you are already enrolled in IRIS contact your consultant or social worker.

Family Care (Inclusia)

Family Care is another State run medicaid program which uses Managed Care Organizations.(MCOs). Family Care offers long term care under one benefit program. Inclusia is an example of a local MCO provider.   Contact the Aging and Disability Resource Center(ADRC) for further information and to see if you quality.  If you are already enrolled contact your consultant/social worker to see if STAR Programs can be added to your care plan.

Children's Long Term Care Support Waiver(CLTS)

The Children’s Long Term Care Support Waiver (CLTS Waiver) is a state Medicare program that helps Children with Disabilities to fund non-medical community based services and supports.  Contact your county social worker to see if you qualify for a waiver, and if STAR programs can be added to your care plan.

 

Challenge Athletes Foundation (CAF) Grants

The Challenged Athletes Foundation (CAF) offers participation and equipment grants to individuals with physical disabilities.  The grant windo opens in the fall (usually Oct) and Closes in Dec. Sports expense grants can be applied for every year and can be used for STAR Membership fees or travel expenses to and from competitions.  You may apply annually.  CAF also has a grant from game of adaptive sports equipment that can be applied for every 3 years if over 18 years of age, and every 2 years if under 18 years of age. We are happy to assist with any documentation for your application.

Insurance Wellness Benefits

Your private insurance or Medicare advantage program my have a community wellness benefit.  Check to see if your plan has a benefit for recreation or fitness memberships as part of their health prevention program.

Scholarships

If you have exhausted all funding sources and financial concerns are still a barrier to your participation at STAR Center, we do have a scholarship program that is a sliding scale based on income and family size.  Please contact us at [email protected] if you are interested in a scholarship.
